Cuyabeno Lodge is asking for a copy of my passport to confirm my reservation - has anyone dealt with this before? This isn't something that I'm very comfortable doing.

It's a common practice, just as any hotel will ask for your passport if you're a traveler. I think it works to your (tax) advantage. It's not like they will steal your i.d. or anything.
I have had to provide passport info for Ecuador, and when in Peru four years ago, we had to present passports they would copy, but not recently. In many places a hotel retains your passport for a bit.
You might ask if there is just some key information you can provide instead of a scanned copy with photograph.

lots of places will ask for a copy, I wouldn't worry about it.
I double checked, and it is indeed for tax purposes when someone is booking from outside the country; if the lodge can provide proof that the person booking is a foreigner, they will not have to charge you a VAT.
