I was lucky enough to get a set at the "bar" of Kawatarou, where you confront your dinner swimming in the tank in front of you. Best to know what you are going to get: a sashimi of a live cuttlefish, the body and tentacles of which will then be served as tempura. You definitely get to confront the reality of what you are eating. Service was all in Japanese but you and they figure it out. Along with a small dish and a half-bottle of sake, about $50. Worth every penny.

The shop provide limited number of service lunch including live squid sasimi and Tempra. The cost ranges from ¥2,000 to ¥2.800 depending on the size of the squid. You can observe all the process from the live cage to the dish. This is a kind of art.
Many foreign people are there and the menu is in English but waitresses speak mainly Japanese. Do not recommend to the people who is concern about killing squids.

We didn't have a dinner reservation so had to go back later to be seated, but it was worth the wait. If you have to wait, I recommend you study their menu before being seated so you can order efficiently. They are very busy so the wait staff won't patiently wait for your order while you look at the menu. Don't expect them to speak English.

If you like fresh sashimi, you will like this place. Squid of various sizes are swimming in the pond in front of you and the chef will catch one depending on your order size and then clean it and slice it. Some will spray the ink so watch out! The squid is still moving when it is served and it is transparent unlike not so fresh one. Whatever you can't finish, they will make into tempura for you along with some vegetable. We ordered a fish sashimi and that, also, was still moving. There is a separate fish pond in the kitchen. I'd think it is better not to do the course meal and just be adventurous.

For our last night in Fukouka, we couldn't get any restaurant we want. Based on hotel's recommendation and the TripAdvisor review, we booked Kawatarou, the original store. Interestingly, as a highly rated restaurant, its staff barely spoke English. As I didn't want the set menu, which was the big mistake, we went through a lengthy ordering process with our waiter. An hour and half later, while we still waited for two last dishes, I decided to add one sushi dish same as our next table had. Our waiter took the order but came back later with lots of explanation. I couldn't quite understand but figured the fish ran out and he wanted us to change. The manager showed up a few minutes later to ask us whether we want sushi and tried to persuade us to get a standard combo. When I pointing to the sushi on the next table, he eventually told us he has only similar fish not the same one. I told him in that case we only want to finish what we have ordered and ask him to follow up. Ten minutes later, he came back, with the bill (not the order but the bill which I didn't even ask for) and told us that all of our orders were served. We went through another round of sign language to indicate from the menu, which two dishes were missing. Finally I gave up and told him that we don't want the dishes anymore. His next words were " please pay the bill". No hot towel, no hot tea, just kicked us gaijins straight out. What an eye opening experience!

We went there on a slow monday for lunch, our hotel told us we dont need to reserve a table beforehand. Just went over and wait, it just took us 15 minutes to get our seat. Better sit by the pool with all the actions are. The food is exceptionally good, one of the best sushi restaurant we have ever been in.
